Why was my tax return rejected by the IRS? What does the error code mean? Below are some of the common Federal Rejection Errors that you may see if your tax return was rejected by the IRS. Solutions and suggestions follow each error description.

FD 010 All alphanumeric fields must contain the type of date specified by the IRS. The most common FD 010 errors refer to the following forms:

Form 1040...
Dependent Name Fields: The dependent's name cannot have special characters or numbers in it.

Solution: Do not use zeros in place of the letter "O" and do not spell words with the letter "O" using a zero.

Solution: Only Roman Numerals are valid. A name like "John Doe III" must have Roman Numerals to designate "third". Do not spell "third", or use the number "3".

Non-Paid Preparer Code: Leave this field blank.

Form W2 ... Box B - Employer ID Number - This number must be 9 digits and entered without dashes "-" or other special characters. If the Employer ID number on the Form W-2 does not have 9 digits, contact your employer. Box C - Employer State and Employer Zip Code - These must be valid state abbreviation codes that match the zip code indicated. Please check and make sure that you are using the correct state abbreviation code for your state.

Schedule C Part V, Other Expense Type (1-9) - Please do not use  special characters like "%", "&", "-" etc. They are not allowed in any of these fields.

Schedule SE SSN on top of Schedule SE - The Social Security Number which appears on the top of Schedule SE must be a valid SSN of the self employed person in question. No dashes or other special characters are allowed.

Margin Item by Line 3 - The only valid entry for this field is EXEMPT-NOTARY. Otherwise this field should be blank.

FD 500, 501, 502, 503, 504, 507 IRS Master File SSN/Name Control Mismatch Errors - Upon receiving an electronically transmitted tax return, the IRS computers compare the taxpayer's, spouse's, and dependents' names against a list of all known Social Security Numbers to validate the tax return. If a match cannot be found between the last name on the tax return and a Social Security Number from the IRS Master File, the tax return will be rejected. In most cases, you need to verify the Social Security Number with a Social Security ID card. It is possible that the tax return was transmitted to the IRS with an incorrect SSN. After correcting the tax return, it can be transmitted to the IRS again. SSN Mismatch errors are common in situations where a last name has recently changed, possibly due to marriage or divorce. You must contact the Social Security Administration at 1-800-772-1213 to update your name, or else you can use the name as it appears on the mailing label of the tax return. This may take awhile, so it is suggested that you file your tax return on paper.

Error Code 501 - Dependent SSN and Year of Birth.

On Sch. EIC, the qualifying SSN, year of birth and the corresponding name control must match information received from the Social Security Administration.

Error Code 502 - Employer EIN Number.

Someone else has used this person's SSN on another tax return or it was transmitted twice.

FD 522 Taxpayer's Date of Birth and FD 523 Spouse Date of Birth in the summary record of an on-line tax return must match the IRS Master File. Taxpayer's Date of Birth and FD 523 Spouse Date of Birth in the summary record of an on-line tax return must match the IRS Master File.

Enter the correct birth dates on the form. You should enter the birth dates in MM DD YYYY format. If you have entered this information correctly then the IRS must have incorrect information in their Master File. This can only be corrected by you. Call the Social Security Administration at 1-800-772-1213.
